New Patient Registration
If you move into the practice area, registration forms are available at reception for your completion. As a guide, the practice registers patients who live within four miles from the surgery.
- If you are over the age of 16 and have not been seen for the past 3 years, you may request a health check.
We strongly encourage you to do this as all adults should have their blood pressure checked every 3 years.
- If you are over the age of 75 and have not been seen for the past 12 months, you may request a consultation. We will be very happy to see you
Medical treatment is available from the date of registration. Please contact reception for further information.
Change of contact details
If your contact details have changed, please let us know. If your address has changed, we will require a utility bill, bank statement or council/government letter as proof.

Registering with the practice
You can bring in your ID as listed below and complete the registration forms at the practice, or:
Please print off a registration form and new patient registration questionnaire, fill them out and bring them in with the required ID.
Over 16s and adults: photo ID (passport or driving licence) and proof of address (bank statement, utility bill or tenancy agreement) and hand it in to the practice reception to start your registration process. You will be contacted to come in for a new patient health check. After this, you can book an appointment with a doctor
5-16 year olds: passport or full birth certificate
Under 5s: birth certificate and red book - an extra form will also need to be completed with regards to Childhood Immunisations.
Please ensure you have your NHS number as this will speed up us receiving
your records from your previous GP.
